Communities working at the intersection of health and justice often navigate fragmented systems, competing priorities, and unclear pathways to care. Systems mapping and action planning help bring clarity, alignment, and shared direction to this work.
Through Systems Mapping and Action Planning, TASC’s Center for Health and Justice partners with communities to understand how policies, programs, and decision points interact across justice, public health, and community systems. Together, we identify gaps, strengths, and opportunities for improvement.
CHJ facilitates structured, collaborative processes that engage stakeholders across sectors, including law enforcement, courts, service providers, public health agencies, and community leaders. These efforts result in clear, actionable plans that are grounded in local context and informed by evidence and best practices.
By translating complex systems into practical strategies, CHJ helps communities move from analysis to action, strengthening coordination, improving access to care, and advancing more just, effective, and accountable systems.
